About Breast Augmentation
Breast Augmentation is a surgical procedure that enhances breast size, improves asymmetry, and refines the feminine profile. Breast augmentation procedures are highly customizable and require an eye for detail. Breast augmentation can be performed with an implant, or a combination of an implant and fat transfer from the patient’s own body.
Implant Placement
Based on patient preference and Dr. Tehrani’s recommendation, your breast implant will be placed either above or below the chest muscle.
Implants are placed below the pectoral muscles (submuscular placement) when there is minimum breast tissue. The implant will act as a breast mound to project the natural tissue.
The implant is placed above the pectoral muscles (sub-glandular placement) when there is ample breast tissue. The breast tissue will cover the implant to provide a natural feel and appearance. This method can only be used when there is enough breast tissue available to successfully cover the entire implant.
Incision Methods
Saline-filled or the newly FDA-approved silicone gel implants may be surgically implanted by three standard routes.
Incision placement will vary between patients based on their goals and anatomy, but the standard incision routes are:
Periareolar. This incision makes circular patterns around the areola and along its natural border.
Inframammary. A pattern is created along the natural fold of the breasts.
Transaxillary. This incision is made along the armpits. A thin tube with a camera will be used along with small instruments to guide implants to the breasts.
Also, saline implants may be placed using a transumbilical method. This method is done via the navel. Small incisions will be made along the abdomen and a small camera will also be used to help guide the implants to the chest.
The implant is composed of an outer flexible, silicone shell and filled with either saline or silicone gel. The outer surface may be smooth or textured.
Implants also have various sizes, profiles and shapes to meet the individual needs of each woman. Implants will be discussed further on in this article.
While breast augmentation will enlarge the breasts, the surgery will not alter underlying basic defects in breast shape and form. Major asymmetries may be improved but will not be completely corrected with breast enlargement alone. A noticeable difference in the size, shape or orientation of the two breasts is considered normal and is actually the rule. If breast size and/or nipple position asymmetries are severe, additional procedures to further improve symmetry may be necessary.

Types of Implants
There are many types of breast implants available today. At Tehrani Plastic Surgery we offer many different shapes, projections, and textures to help you achieve your ideal breast. These options include:
Saline breast implants. These implants have a silicone shell and are filled with a saline solution. Saline implants are popular because they provide a realistic breast feel. They are also a safe option. In the event of a rupture, your body will naturally absorb the saline solution.
Silicone breast implants. Silicone implants have a silicone shell. Inside of the shell is a silicone gel.
Gummy bear breast implants. These implants are growing in popularity. They are a silicone shell filled with a gel the consistency of a gummy bear. The great benefit of this implant is the gel it uses. If an implant ruptures, the gel does not leak but retains its form.
These implants will come in a variety of shapes, such as teardrop implants and round implants. Teardrop implants are shaped similarly to a teardrop, thicker at the bottom and thinner on top. This is a popular shape for gummy bear implants. The other option is a round implant. The round implant is the most popular because the shape helps to even out the appearance of the whole breast.
Projections vary from low, medium and high. The thickness of the implants will depend on how much you desire your breasts to improve in volume. A low profile is for women who want minimum improvement and a high profile for women who want maximum change.

Composite Breast Augmentation
An additional treatment or alternative procedure to breast implants is composite breast augmentation. This procedure removes fat from another area of your body, such as the thighs, flanks or buttocks and relocates it the chest. On its own, fat transfer is for women who want minimum enhancement in their breast. If you are interested in receiving more than one cup size in improvement, you will need implants.
Composite breast augmentation is performed in two steps that can be performed within the same appointment or two separate appointments. In your consultation with Dr. Tehrani, you and he will decide on an area of your body to collect fat from. The first step of the composite augmentation procedure requires liposuction. Liposuction is a technique that removes fat from underneath your skin.
Dr. Tehrani will dislodge patients’ unwanted fat with advanced liposuction methods that gently remove fat from the body but leave the fat healthy and intact enough for transfer to the breasts. Once the fat is removed, it will be purified and placed in syringes. In the second step of the procedure, the fat will be injected into the breasts. He will strategically place the fat so that you obtain your best results.

Composite breast augmentation is an excellent solution to breast enhancement. The treatment does not require an allergy test as only your fat is being introduced into the body. Dr. Tehrani will ask about your allergies, past procedures, current medications, and general health. He will walk you through the steps of breast augmentation and go over all the details of the techniques. Together, you and Dr. Tehrani will create a treatment plan that achieves your ideal outcome.
Preparation
Dr. Tehrani will provide you with instructions on how to best prepare for the procedure and recovery period. You will need to acquire prescriptions and arrange for rides prior to the treatment. Someone will need to drive you to your appointment and take you home as well. It is recommended to have someone stay with you the first night of your recovery.
You will need to stop smoking and taking antihistamines a few weeks before the surgery date.
Twelve hours before your surgery you will need to fast from everything except water. We suggest you arrive at your appointment a few minutes early to help ensure you have a relaxing morning.
Procedure Description
Breast Augmentation is usually performed under general anesthesia or conscious sedation with local anesthesia.
The details of your procedure will vary based on your personal goals and the need of your individual physique.
When you arrive at your procedure, your chest will be cleaned and marked for surgery. Incisions will be made in predetermined areas. Your choice of implants will be placed within your breasts to enhance your natural features. Saline or silicone implants may be placed either above or below the chest muscle. Sutures will be layered deep within the breasts to help them adjust to their new contours. Once completed, the incisions will be closed and bandaged. Surgical scar(s) are usually hidden as much as possible in skin folds.
Breast Augmentation Recovery
Discomfort during recovery from a breast augmentation will be controlled with oral medications and long-acting local anesthesia. A soft bra or compression garment is usually worn for several days postoperatively.
Patients are usually discharged to the care of a responsible adult to recover at home. All sutures are usually removed in one to two weeks at follow-up appointments.
Light activity may be resumed in seven days. However, sports activities will need to be resumed in three to six weeks depending on the surgeon.
Breast Augmentation Recovery
- Discomfort is controlled with oral medications and long-acting local anesthesia.
- A soft bra or compression garment is usually worn for several days postoperatively.
- Patients are usually discharged to the care of a responsible adult and recover at home.
- All sutures are usually removed in 1-2 weeks.
- Light activity may be resumed in 7 days. Sports activities may be resumed in 3-6 weeks (depending on your surgeon).
Results
You will immediately notice the results of your breast augmentation. Your breasts will appear larger, shapelier, and more symmetrical. The procedure will correct size differences between the breasts as well. As swelling decreases and your body adjusts to the new size, you will notice the results get even better. Final results should set in after a couple of months.
Patients who receive fat transfer to the breasts will see their final results as their body accepts the transferred fat cells. It is normal for the body to accept 70 to 80% of transferred fat. Dr. Tehrani takes this into account when transferring fat to the breast so that patients will see their ideal results after the body has accepted the transfer. The remaining percentage of the transferred fat is safely accepted back into the body.
